Benton County Death Records: Local Health Unit

The Benton County Health Unit is the local point of contact for death certificate applications. The unit is located at 1200 West Walnut Street, Rogers, AR 72756. You can call them at (479) 986-1300. Staff there can help you complete an application and will forward it to the Arkansas Department of Health in Little Rock for processing. The certificate is then mailed directly to you. This is a useful option if you prefer to handle paperwork in person rather than mailing documents to a central state office.

The health unit serves residents across the entire county. Hours of operation follow the standard state health unit schedule. If you are unsure whether your request qualifies, the staff can tell you what documentation you need to bring, including acceptable forms of photo ID. Keep in mind that the unit does not hold copies of certificates on-site. All certificates come from the Division of Vital Records.

Note: The Benton County Health Unit accepts applications but does not issue certificates on the same day. Processing takes place at the state level after the unit forwards your paperwork.

You have four ways to request a certificate. In person at the Little Rock office is the fastest if you can make the trip. By mail, send a completed application, a copy of your photo ID, and a check or money order payable to Arkansas Department of Health. Online orders go through VitalChek, which adds a $5.00 processing fee and a $1.85 identity verification fee to the base certificate cost. Phone orders are also accepted. The first copy of a death certificate costs $10.00. Each additional copy ordered at the same time is $8.00. If no record is found, the $10.00 search fee is still charged and is not refunded.

Records less than 50 years old are restricted under Arkansas Statute 20-18-305. Only immediate family members, legal representatives, or approved researchers can access those records. Records 50 years old or older are public and open to anyone who pays the fee.
